    NEWS & INFORMATION

    FORMS

    • Physical Exam and Parent History Form

    • Scoliosis - Please visit infosnap to complete it online or download the form and return to the school nurse.

    • Asthma Action Plan - Complete only if your child has asthma. No other form will be accepted.

    • Food Allergies - This form is required if your child has any food or insect-related allergy-to which medication may be needed. Please make sure you have your physician sign the form and return it to the school nurse along with one dose of medication.

    • Medication in School - Only complete if your child needs a prescription or over-the-counter (OTC) medication. The parent must return the form to the nurse along with the prescription bottle or small unopened bottle of OTC medication.

    MEDICATION

    If it is necessary for a student to receive medication during school hours, parents must comply with New Jersey State Narcotic and Dangerous Drugs Law; Title-24:21--16-18...

    1. Parent or guardian must provide a written request for the administration of prescribed medication at school.

    2. Written orders must be provided by the doctor, telling what condition the medication treats, side effects, and how often it is to be administered.

    3. Medication containers must be properly labeled by a pharmacy or doctor and brought directly to the school nurse by the parent/guardian upon entering the school building.  All medication must be in its original container. Over-the-counter medication must be in a UNOPENED bottle. 

    4.  No medication over the counter (including and not limited to; Aspirin, Tylenol, and others) will be administered without a physician's and parents' note.  Students are NOT permitted to carry medication on their person or in their locker.  This includes prescriptions or over-the-counter.

    5. Students are not permitted to self-medicate unless it is for a life-threatening illness and the above noted paperwork is on file with the nurse.

    PLEASE NOTE THE CHANGE IN THE SCOLIOSIS LAW
    N.J.A.C. 6A: 16-202(f), N.J.S.A 18A: 40 - 4.3 to 4.5 & A-1183 Pamphlet Law 200c 126 9/15/02. Every board of education must provide a biennial examination of every student between the ages of 10-18.
    Beginning in 2003-2004, screening will be done in 4th, 6th, 8th, and 10th grades.|
