    All incoming 7th grade students need to complete medical forms.  Please see "FORMS" below for details.

    Dear Parents and Students, 

    I would like to take this opportunity to welcome you to W. R. Satz School. I hope everyone had a happy and healthy summer. I look forward to a great school year. I have compiled a few health related issues that will help you navigate through school.

    We have made a slight change in the required forms for entering Satz School with the hope of saving parents time. This new change will help streamline paperwork for all new to the district, and incoming 7th grade students.  The Confidential Health Appraisal and Health Examination Form for New Students will be replaced with the Physical Examination Form and the History Form.  These two forms are components of the Sports Physical Packet. In addition, please ask the doctor to provide the school with a list of your child’s current immunizations.

    If your child intends to participate in any school sport, he/she must visit Holmdel's Athletic Department's website and create an account in order to register for any sport.  Make sure to download the Sports Packet on the home page.  The Sports Physical must be completed by a physician and the History Form by a parent/guardian.  These 4 pages must then be uploaded to the athletes' account.  An e-mail will be sent to the nurse informing her that your child would like to participate in a sport and she will then review all of the information.  The parent/guardian will then get an e-mail informing them their child has been cleared or something is missing, or needs review. Please keep in mind you must update Step #6 with each new sport your child chooses to try-out for and Physicals are only good for 1 year from the date the physician actually performed the examination.

    State regulations prohibit school nurses from administering any unauthorized medication to your child.  This includes over the counter cough medicines, antacids, Tylenol, Advil, essential oils and topical ointments. Students are not permitted to carry or self-administer any medication unless it is authorized by their physician and is for a life-threatening illness. If you anticipate your child needing medication during the school day, please visit my website and download a Food Allergy Action Form, Asthma Action Plan, or Authorization for Medication to be taken during school hours.  Section A should be completed by the parent and Section B needs to be completed, signed and stamped by the physician.  The medication needs to be given to the school nurse along with the completed form.  All over-the-counter medications need to be in their original unopened containers and prescription medications should be labeled with the student’s name, correct medication, dosage, and administration time.

    If your child is sick, please keep them home 24 hours after their symptoms have resolved and their fever is lower than 100 degrees without fever reducing medication, i.e. Tylenol (acetaminophen) or Advil (Ibuprofen). Even if your child tells you “I’m fine” or “I have to take a test”, do not allow him/her to return to school.  Returning to school sick only exposes others to the illness and continues the cycle.

    NEWS & INFORMATION

    FORMS

    • Physical Exam and Parent History Form - When entering Satz School, please complete both forms.

    • Scoliosis - All 8th graders are required by the State of NJ to be screened for scoliosis biennially. Please visit infosnap to complete it online or download the form and return to the school nurse.

    • Asthma Action Plan - Complete only if your child has asthma. No other form will be accepted.

    • Food Allergies - This form is required if your child has any food or insect-related allergy-to which medication may be needed. Please make sure you have your physician sign the form and return it to the school nurse along with one dose of medication.

    • Medication in School - Only complete if your child needs a prescription or over-the-counter (OTC) medication. The parent must return the form to the nurse along with the prescription bottle or small unopened bottle of OTC medication.

    • Sports Registration - All athletes must create an account in order to register for a sport.If you have already created an account, please make sure you revisit Step #6 to complete "Eligibility" before each sport that your child would like to participate in. 

    MEDICATION

    If it is necessary for a student to receive medication during school hours, parents must comply with New Jersey State Narcotic and Dangerous Drugs Law; Title-24:21--16-18...

    1. Parent or guardian must provide a written request for the administration of prescribed medication at school.

    2. Written orders must be provided by the doctor, telling what condition the medication treats, side effects, and how often it is to be administered.

    3. Medication containers must be properly labeled by a pharmacy or doctor and brought directly to the school nurse by the parent/guardian upon entering the school building.  All medication must be in its original container. Over-the-counter medication must be in a UNOPENED bottle. 

    4.  No medication over the counter (including and not limited to; Aspirin, Tylenol, and others) will be administered without a physician's and parents' note.  Students are NOT permitted to carry medication on their person or in their locker.  This includes prescriptions or over-the-counter.

    5. Students are not permitted to self-medicate unless it is for a life-threatening illness and the above noted paperwork is on file with the nurse.

    SCOLIOSIS NOTICE

    PLEASE NOTE THE CHANGE IN THE SCOLIOSIS LAW
    N.J.A.C. 6A: 16-202(f), N.J.S.A 18A: 40 - 4.3 to 4.5 & A-1183 Pamphlet Law 200c 126 9/15/02. Every board of education must provide the biennial examination of every student between the ages of 10-18.
    Beginning in 2003-2004, screening will be done in 4th, 6th, 8th, and 10th grades.|
